This policy explains what information we collect through renewfertilityandhormones.com, our webinars, and our booking and contact forms, how we use it, and the choices you have.

1. Information we collect

Information you give us. This includes your name, email, phone number, and anything you share when you book a discovery call, join our waitlist, register for a webinar, or send us a message, such as the reason you are reaching out.

Payment information. Payments are handled by our payment processors. We do not store your full card number.

Information collected automatically. When you visit our site, we and our tools may collect your IP address, browser type, device, pages viewed, and how you found us. We collect this through cookies and similar tools.

We use your information to:

• Respond to your questions and schedule your discovery call or appointment
• Send you webinar access, waitlist updates, and appointment reminders
• Provide care if you become a patient

• Send you other messages you agreed to receive

3. Text messages and email

If you give us your phone number and check a consent box, you agree to receive text messages from RENEW Fertility & Hormones. Depending on the box you check, these may include appointment scheduling, reminders, replies to your inquiry, and marketing messages. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for help.

We do not sell or share your mobile number or text messaging consent with third parties for their marketing purposes.

You can unsubscribe from our emails at any time with the link at the bottom of each email.

We do not sell your personal information.

We share information only with trusted service providers who help us run our practice. These include our website and CRM platform (GoHighLevel), our electronic health record and patient portal, payment processors, email and file storage (Google Workspace), our supplement dispensary (Fullscript), and lab partners such as Quest and LabCorp when labs are ordered. These providers may only use your information to provide services to us.

We may also share information if the law requires it, to protect someone's safety, or if our practice is sold or merged.

5. Cookies and ad tracking

We use cookies and similar tools to understand how people use our site and to show our ads to people who may be interested. These tools may include Google Analytics, Google Ads, and the Meta Pixel.

You can block or delete cookies in your browser settings. You can also opt out of personalized ads through your Google and Meta ad settings, or at optout.aboutads.info.

6. Health information

We know the information you share about your health is personal. Please do not share detailed medical information through our website forms. Our care team will collect your health history securely once you become a patient.

If you become a patient, your health information is protected under the federal HIPAA law and described in our Notice of Privacy Practices, which we provide to you as a patient and which is available on request. We do not use your health information for advertising, and we do not sell it.

7. How we protect and keep your information

We use reasonable safeguards to protect your information, such as secure platforms, access controls, and encrypted payment processing. No system is 100% secure, so we cannot promise perfect security.

We keep your information as long as we need it to provide our services and meet legal, medical record, and tax requirements. After that, we delete it or make it anonymous.

You can ask us to:

• Correct information that is wrong
• Delete your information, unless we need to keep it by law
